WebOntario Health Teams (OHTs), i which are intended to help improve health-system integration and performance across a range of outcomes linked to the quadruple aim1ii,iii. The movement to integrated care by way of OHTs represents some of the most rapidly evolving health-system reform that Ontario has experienced in more than a decade.
